About Elevate

Elevate owns and operates its agencies as well, currently operating in northeast Florida. Elevate was started by two previous colleagues with extensive experience and knowledge in the home health market.


Elliot Kohn

Elliot Kohn, CEO and Managing Partner has been in the Home Health world for ten years, most recently with Promise Care NJ in Jersey City, NJ for the last five years, where he held the title of CFO as well as CMO. Elliot was responsible for overseeing the Finance, Business Development, and Intake departments. With new policy and strategies, Elliot successfully doubled referrals and census within the last few years. Promise Care was recognized as a Home Health Elite Agency for three years straight, which is awarded to the top 500 agencies in the country. Elliot has earned the reputation of someone who is genuinely committed to the care and services provided to the patients the agencies service.


Steven Schwartz

Steven Schwartz, President and Managing Partner previously with Revival Home Health Care in Brooklyn, NY for 16 years, held various roles going up the ranks to the Administrator for the last six years leading Revival, and its sister agencies, as one of the most prestigious agencies in NYC with over 30 million in revenue per year. Steven was at the forefront of implementing essential programs that set Revival ahead from the rest, including rehospitalization prevention programs that reduced their rate well below its competitors and deficiency-free surveys and more, establishing solid roots for the ultimate care provided under his leadership.

Steven and Elliot joined forces and founded Elevate Home Health Group after several discussions over the past couple of years with a strong support team for management services as well as owning and operating in various states on the eastern seaboard.

Their first acquisitions were in Orlando and Jacksonville, Florida while seeking opportunities in Virginia, North Carolina, South Carolina, Georgia, Tennessee, and Maryland. By leveraging existing relationships across those states and creating synergies between other ancillary companies serving the homebound seniors that require services at home, patients and families will have a closer continuum of care.
