Rehabilitation Therapies
When you’re recovering from joint replacement surgery or after a stroke, or if you’re living with a chronic condition that makes it difficult to walk or dress yourself, you’ll need help to regain strength, coordination, and balance. The best place to do this is where you feel most comfortable, and that’s why rehabilitation therapists from Elevate come to your home.
Physical Therapy (PT)
- Restore mobility/strength
- Teach Transfer and walking techniques
Occupational Therapy (OT)
- Improve/restore function in ADLS (Assist of Daily Livings) Eating, bathing, dressing, household tasks, toileting.
- Cognitive retraining
Speech-Language Pathology (SLP)
- Develop/restore speech production (as a result of trauma, surgery or stroke) or implement functional communication system
- Restore safe swallowing function
Social Work
- Evaluate social and emotional factors that may be barriers to treatment and/or improvement
- Assist with entitlements
- Provide counseling
- To help improve compliance
- To provide patient and/or caregiver support
- To develop effective coping skills
Advance Care Planning
- Provide enhanced clinical assessments of advanced disease, if appropriate, to better identify palliative or hospice needs
- Educate patient and family on advanced care planning, including advanced directives
- Facilitate meaningful conversations with family and providers about goals and care
Who is a candidate for Elevate Home Care?
Patients who meet or more of these criteria:

Recuperating from acute illness or disabled

Depressed or cognitively impaired

Post-op surgical care need

Complex medication regimens

Somewhat diminished capabilities but elects to live independently at home pr an ILF

Needs assistance with ADLs (Activities of Daily Living) with RN supervision

Homebound

Homebound patient
For Medicare and some commercial/managed care insurance, the patient must be deemed homebound to receive home healthcare services. A “homebound” patient is unable to leave the home without what’s considered a “taxing effort” (physical or mental) and needs intermittent skilled nursing and/or Therapy

Remote Patient Monitoring (RPM)
Elevate is working with the most advanced RPM provider in the country working in sync with your primary care physician. Each eligible patient will receive a smart device that will be monitoring you and your health 24/7 and reminding you to take your medication, blood pressure, etc. Telehealth video visits can be done directly from the device easy and efficiently.
